چکیده

background: although the positive effects of cardiac rehabilitation (cr) for patients with heart failure have been known, only a small proportion of such patients have been referred for comprehensive cr. the aim of this study is to compare the extent to which a cr program improved the exercise capacity and modifiable risk factors of patients with and without impaired systolic function. methods: this study was an observational study undertaken in imam ali hospital in kermanshah (2001-2006), the only referral center for cr in the west of iran. 502 patients, who completed cr program, were included in this study. they were divided into two groups: impaired systolic function with  ejection fractions (ef) <50% (n=185) and normal systolic function (ef≥50%, n=317). results: both groups were comparable in terms of all baseline characteristics such as bmi, hypertension, smoking and diabetes. there were significant improvements in  exercise capacity, bmi, waist circumference, total cholesterol, ldl, triglyceride, systolic and diastolic blood pressure and depression score in both groups after cr (p<0.001). however, the effect of cr on hdl did not reach a statistical significant level in either group. in addition further analyses showed that there was no statistically significant differences between patients with severe impaired systolic function (ef<35%) and normal ef, however changes in lipid profiles were not stastically  significant.  conclusion: this study confirmed that the effects of cr on improvement of exercise capacity and risk profiles are almost equal in patients with and without systolic dysfunction.

The Impact of Cardiac Rehabilitation on Pulmonary Artery Systolic Pressure and Left Ventricular End-Diastolic Pressure in Patients after Coronary Artery Bypass Graft Surgery

Background:Cardiac rehabilitation program (CRP) is a useful method of modifying cardiovascular risk factors, improving life expectancy and quality of life in patients with ischemic heart disease (IHD). The present study was conducted to evaluate the effects of cardiac rehabilitation on the pulmonary artery systolic pressure (PASP) and left ventricular end-diastolic pressure (LV...

Exercise capacity in patients with severe left ventricular dysfunction.

Normal or near-normal exercise capacity has been thought to reflect normal left ventricular function. Many compensatory mechanisms could preserve exercise capacity in patients with severe left ventricular dysfunction. We evaluated exercise capacity using a treadmill exercise test in 26 patients with severe left ventricular dysfunction defined by a left ventricular ejection fraction of 30% or le...

Contribution of exercise-induced mitral regurgitation to exercise stroke volume and exercise capacity in patients with left ventricular systolic dysfunction.

BACKGROUND Functional mitral regurgitation (MR) is common in patients with heart failure and left ventricular (LV) dysfunction, and its severity may vary over time, depending primarily on the loading conditions. Because dynamic changes in the severity of functional MR may affect forward stroke volume, we hypothesized that exercise-induced changes in MR severity influence the stroke volume respo...

Left ventricular performance during muscular exercise in patients with and without cardiac dysfunction.

THE RELATIVE CHANGES in cardiac output and total body oxygen consumption (V02) which occur during muscular exercise provide important information concerning the overall ability of the cardiovascular system to meet the demands of this common form of stress. It is evident, however, that when a less than normal rise in the cardiac output occurs relative to a given increase in V09, it cannot necess...
